I think there are two major problems. First, parents don't teach their children. They think they can dump them on other people or the system to take care of. Second, people have the misconception that if someone can drive from point A to point B and not get in an accident, they must be a good driver. I see people every day while I am commuting that don't get in accidents, but many either cause them or another driver has to take action to prevent an accident. A good example is old people, just watch one some time. I have been driving since well before the law said I should, I drive my sports cars in the snow, I race autocross on the weekends, and the tracks when I have time and money. Every time I race I know I'm learning a little more, and what I'm not learning I am programming as a reflex. No one is a perfect driver, but it is like any other form of training, you can make yourself better.
